Twin Cobra is a vertical shoot-em-up where the player controls an assault helicopter. The objective is to infiltrate the nation of Kaban and destroy the military base of its leader, Commander Anziga.
Enemy forces include various ground and air units such as tanks, stationary guns, and helicopters. Defeating larger helicopters yields power-up icons, which change the helicopter's primary weapon. These colored icons correspond to specific firing patterns: a forward spread, a four-way shot, a wide laser, or a continuous laser beam.
Progress is organized into sections, each concluding with the player landing on a carrier ship. A bonus tally is presented at these intervals.
