Twin Cobra is a vertical shoot-em-up where the player controls an assault helicopter. The objective involves infiltrating the nation of Kaban to destroy the military base of its leader, Commander Anziga.
Enemy forces include various military units such as tanks, small helicopters, and stationary guns. Defeating larger helicopters yields power-up icons, which change the helicopter's weapon when collected. These colored icons correspond to different firing patterns: a forward spread, a four-way shot, a wide laser, or a continuous laser.
Progress is organized into sections, each concluding with the player landing on a carrier ship. A bonus tally is calculated at these points.
