"Realm" is set in a future where Earth has been nearly destroyed by an alien attack, with its cities left in ruins. The player controls a Biomech cyborg, representing humanity's final chance for survival, on a mission to eliminate the alien threat across five distinct realms.
As a side-scrolling action title, the game involves navigating through stages and confronting a variety of enemies. The cyborg is equipped with an arsenal including lasers, heat-seeking missiles, and plasma boomerangs. Progression requires defeating large boss characters that guard the advance to subsequent levels.
Throughout the stages, players can collect power-ups. These items provide temporary defensive shields or enhance the capabilities of the cyborg's weaponry.
