"George Foreman's KO Boxing" is a sports simulation where the player controls the heavyweight champion through a twelve round tournament to claim the championship belt. The perspective is third-person, positioned behind the boxer, allowing for lateral movement to evade attacks.
Combat involves throwing standard crosses and, by holding up, jabs. Successfully landing punches and avoiding damage builds a challenging superpunch that deals significant harm. Each match is structured in three minute rounds, with a maximum of three rounds total. A boxer's condition is tracked by an energy meter that depletes when hit and partially refills between rounds and after knockdowns.
The match concludes under specific conditions: a technical knockout after four total knockdowns, or a regular knockout from three knockdowns in one round. When knocked down, rapid button presses are required to stand. Progress can be saved via a password system, and most versions support two-player competition.
