Streets of Rage 3 is a beat 'em up where players confront the returned antagonist Mr. X, whose scheme involves substituting robots for city officials. The playable roster includes series regulars Axel, Blaze, and Skate, alongside the new character Dr. Zan.
Gameplay introduces a special move power bar; executing these moves consumes energy only when this bar is not completely filled. Character-specific interactions with weapons are another addition, such as Skate having a special maneuver with a knife but not a pipe, while Axel's capabilities are the reverse. Dr. Zan converts any acquired weapon into throwable energy balls.
The inclusion of secret characters and multiple possible endings encourages repeated playthroughs.
