Ms. Pac-Man is a maze-chase game where the player controls the title character through neon environments. The primary objective is to clear each stage of all dots while evading four distinct ghosts: Blinky, Pinky, Inky, and Sue. Contact with a ghost costs a life, and rounds conclude when the maze is emptied.

Strategic navigation is central, requiring players to account for variable corridor widths, dead ends, and the placement of side tunnels. These tunnels can affect ghost speeds and provide escape routes, though their effectiveness changes across the game's four different maze layouts. This variety in design makes consistent route planning challenging and encourages adaptive play.

Power pellets called Energizers, located near maze corners, temporarily reverse the threat. When consumed, they turn the ghosts blue, allowing Ms. Pac-Man to eat them for escalating points. This vulnerability period shortens in later stages, and the ghosts' overall speed increases, creating a fair but escalating difficulty curve. Ghost behavior itself is less predictable than in the original Pac-Man, blending pursuit and dispersal patterns based on player position and maze configuration.

Bonus items appear as moving fruit that enters and exits the maze via side tunnels, offering points that increase in value as the game progresses. Brief animated intermissions depict comedic scenes involving Ms. Pac-Man, Pac-Man, and their offspring, Junior. The game supports an alternating two-player mode, and many home versions included adjustable settings for earning extra lives.

While the core mechanics have been preserved across many platforms, specific adaptations often altered visual details or maze designs due to hardware limitations. Later digital re-releases typically restored the original arcade layouts and added features like high score saving and display options.
